Integumentary System Review:<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>the skin (integument) is the largest body organ and functions to:<ul><li>protect against injury (e.g., toxins, mechanical insults, UV radiation)<\/li><li>regulate body temperature<\/li><li>maintain fluid balance (i.e., protect against dehydration)<\/li><li>serve as sensory organ<\/li><li>immune system surveillance against potential threats<\/li><li>synthesize vitamin D<sub>3<\/sub> (cholecalciferol)<\/li><\/ul><\/li><li>the skin is also dynamic in a constant state as the cells making up the outer surface (epidermidis) are continuously being shed and replaced by underlying cells arising from the dermis layer<\/li><\/ul>\n<\/div><\/div>\n<\/div>\n<\/div>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:50%\">\n<div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<p><strong>(a) Epidermidis<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>epidermis is striated squamous epithelium comprised of keratinocytes, which synthesize the fibrous structural protein keratin<\/li><\/ul>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<p><strong>(b) Dermoepidermal junction<\/strong><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<div class=\"wp-block-group\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\n<ul class=\"wp-block-list\"><li>i.e., basement membrane joining the epidermidis and dermis layer<\/li><li>basal layer cells are connected via adhesion proteins (desmosomes) and intracellular keratin cytoskeleton <\/li><\/ul>\n\n\n\n<p><strong>II. <u>Infections of the Integumentary System<\/u><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>referred to as skin and skin tissue\/structure infections (SSTI\/SSSI), now more often called acute bacterial skin and skin structure infections (ABSSSI) \u2192 distinguish between bacteria, fungal, and viral SSSIs<\/li><li><strong>uncomplicated SSSI (uSSSI\/uABSSSI):<\/strong> localized and confined to the epidermis and dermis layers<ul><li>e.g., impetigo, folliculitis, furuncles, erysipelas, superficial abscesses, wound infections, uncomplicated cellulitis<\/li><li>generally caused by&nbsp;<span class=\"has-inline-color has-vivid-purple-color\"><em>S. aureus<\/em>&nbsp;<\/span>and&nbsp;<span class=\"has-inline-color has-vivid-purple-color\"><em>S. pyogenes<\/em> (Group A)<\/span> \u2192 commensal bacteria<\/li><\/ul><\/li><li><strong>complicated SSSI (cSSSI\/cABSSSI):<\/strong> infection of deeper skin structures (e.g., fascia, muscle) that requires surgical intervention or occurs in the presence comorbidities (e.g., HIV, diabetes mellitus)<ul><li>e.g., complicated cellulitis, necrotizing fasciitis, myositis, gas gangrene, major abscesses, infected burns and ulcers, infected bite wounds, diabetic foot infections<\/li><\/ul><\/li><\/ul>\n<\/div><\/div>\n<\/div><\/div>\n<\/div><\/div>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" leprae<\/em><\/li><li>uncomplicated cellulitis \u2013 \u03b2-hemolytic <span class=\"has-inline-color has-vivid-purple-color\">streptococci (groups A, B, C, G F)<\/span> &gt; <em><span <\/strong>(subcutaneous tissues, connective tissues, fascia)<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>complicated cellulitis \u2013 <em><span class=\"has-inline-color has-vivid-purple-color\">S. anaerobes<\/li><li>pyomyositis \u2013 <span class=\"has-inline-color has-vivid-purple-color\"><em>S. aureus<\/em>* <\/span>including <span class=\"has-inline-color wp-block-group-is-layout-flow\">\n<p><strong>(c) Clinical Microbiology of<\/strong><strong> SSSIs<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>specimens are obtained from exogenous or endogenous wound or abscess sites suspected of infection<\/li><li>specimens obtained by swab, removal of purulent material (e.g., pus), or needle aspiration<\/li><li>in clinical microbiology reports, cell counts are semi-quantified \u2192 1+ = cells rare in number; 2+ = moderate; 3+ = many; 4+ = great many<\/li><li>wound infections are usually polymicrobial<\/li><\/ul>\n\n\n\n<p><strong>III. <u>Superficial Skin Infections (Uncomplicated)<\/u><\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>uSSSIs that are localized (non-systemic) infections of the epidermidis and dermis<\/li><li><span style=\"text-decoration: underline\">pathophysiology<\/span>: bacteria invade through a break in skin (e.g., trauma, insect bite, cutaneous disease); colonizes local tissue (e.g., folliculitis, local abscess) or spreads at the site of inoculation (e.g., erysipelas, cellulitis)<\/li><\/ul>\n<\/div><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-column extremities<\/li><li><span style=\"text-decoration: underline\">epidemiology:<\/span> 10-20 cases per 1000 pts-yr; most common in children, elderly, and areas with poor sanitation<ul><li>signs\/symptoms: red sores w\/ pus that may rupture and cause crusty skin; fevers, chills, malaise<\/li><li>risk factors: poverty, crowding, poor hygiene; occurs usually in warm humid conditions<\/li><li>pathogens: established by skin trauma (e.g., insect bite)<\/li><\/ul><\/li><li>impetigo contagiosa is caused by <em>S. pyogenes<\/em>; most common in children; contagious disease<\/li><li>bullous impetigo (purulent) is cause by <em>S. aureus<\/em>* due to exfoliatin release<\/li><\/ul>\n<\/div><\/div>\n<\/div><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-column (e.g., shaving) or blocked (e.g., sweat, makeup).<\/li><li><span style=\"text-decoration: underline\">signs\/symptoms<\/span>: rash, itching, pimples\/pustules<\/li><li>pathogens &#8211; <em><span class=\"has-inline-color has-vivid-purple-color\">S. aureus*<\/span><\/em>, <em><span class=\"has-inline-color has-vivid-red-color\">P. aeruginosa<\/span><\/em> (under chlorinated hot tubs), <em>Propionibacterium acnes <\/em>(acne vulgaris)<\/li><\/ul>\n\n\n\n<p><strong>(c) Furunculosis (boils), Carbunculosis (cluster of boils)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>furuncles (boils) are singular swollen skin infection of hair follicle; often complication of folliculitis<\/li><li>carbuncles are clusters of highly swollen follicular masses.&nbsp;<\/li><li>signs\/symptoms: redden nodule initial (furuncle), no fever; swollen, deep, painful follicular mass; fever, chills, and malaise due to bacteremia common for carbuncles<\/li><li>usually occurs on back of neck, face, armpit, and buttocks<\/li><li><em>S. aureus <\/em>most common pathogen; <span class=\"has-inline-color has-vivid-purple-color\">CA-MRSA<\/span> causes \u201cspider bite-like\u201d lesions with necrotic centers<\/li><\/ul>\n\n\n\n<p><strong>(d) Skin Abscess<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>abscesses are collections of pus within the dermis and deeper skin tissues; often occurs after boil develops or skin trauma that seeds the infection<\/li><li>common SSSI in IV drug users (watch for <span class=\"has-inline-color has-vivid-purple-color\">MRSA<\/span>).<\/li><li>characterized by localized accumulation of neutrophils in response to tissue necrosis; large bacteria numbers (e.g., 4+) are observed in purulent remove for lab diagnostics<\/li><li>pathogens: <span class=\"has-inline-color has-vivid-purple-color\"><em>S. aureus<\/em> (MRSA)<\/span> but, other microflora species may be involved<\/li><li><span style=\"text-decoration: underline\">signs\/symptoms<\/span>: fevers, chills, swelling, harden tissue (induration), redness, fluid drainage<\/li><\/ul>\n\n\n\n<p><strong>(e) Cellulitis<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>cellulitis is an acute inflammation of connective tissue<\/li><li>can manifest in both the dermis and subcutaneous layer<\/li><li>most common of the ABSSSI hospitalizations<\/li><li><span style=\"text-decoration: underline\">epidemiology<\/span>: ~20 cases per 1000 pts-yr.; most common in elderly pts; 40-70% of cases in lower limbs<\/li><li><span style=\"text-decoration: underline\">risk factors<\/span>: disruption of cutaneous barrier, lymphedema, local venous stasis, chronic edema, obesity, previous episodes<\/li><li><span style=\"text-decoration: underline\">pathogens<\/span>: \u03b2-hemolytic streptococci (groups A, B, C, G F) &gt; <em>S. aureus<\/em> &gt;&gt; <em>H. influenzae b, V. vulnificus, C. perfringens, B. anthracis, Erysipelothrix, Enterobacteriaceae, P. aeruginosa<\/em><\/li><li><span style=\"text-decoration: underline\">predisposing factors<\/span><strong>:<\/strong> spider bites, blisters, tattoos, dry skin, eczema, animal bite, surgery, IV drug users, pregnancy, diabetes\/obesity (affect blood circulation \u2192 hinder innate neutrophil immunity)<\/li><li><span style=\"text-decoration: underline\">signs\/symptoms:<\/span> swelling, tenderness, warmth and\/or redness (erythema) of infected area; fever, chills, malaise<\/li><li>cellulitis causes 25-30K deaths in the U.S. each year, many are elderly or immunocompromised<\/li><li>microbiology: identifying the pathogen is uncommon for uncomplicated cellulitis<\/li><\/ul>\n\n\n\n<p><strong>&nbsp;(f) Erysipelas (St. Anthony\u2019s fire)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>cellulitis of skin involving blockage of skin and dermal lymphatics; 80% of cases in lower limbs<\/li><li>can manifest in both the dermis and subcutaneous layer<\/li><li><span style=\"text-decoration: underline\">epidemiology:<\/span> 0.1-2.0 cases per 1000 pts-year; most common in elderly persons; female predominance<\/li><li><span style=\"text-decoration: underline\">pathogen:<\/span> <em>S. pyogenes<\/em> (GAS)<\/li><li><span style=\"text-decoration: underline\">signs\/symptoms<\/span>: bright red painful lesions, burning pain, raised borders, flu-like symptoms<\/li><li><span style=\"text-decoration: underline\">epidemiology<\/span>: infants, young children, elderly, patients with nephrotic syndrome<\/li><li><span style=\"text-decoration: underline\">microbiology:<\/span> specimen obtained by aspiration of lesion<\/li><\/ul>\n\n\n\n<p><strong>IV. <\/strong><strong><u>Soft Tissue\/Skin Structure<\/u><\/strong><strong><u><\/u><\/strong><\/p>\n\n\n\n<p><strong>(a) Infected bites<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>cellulitis and deep SSTI can occur after animal or human bite.<\/li><li>infection often polymicrobial involving mouth microflora bacteria<\/li><li>bite wound associated <em>Haemophilus<\/em><\/li><li>cats\/dogs \u2013 <em>Pasteurella multocida, Capnocytophaga canimorsus, <\/em>oral anaerobes (<em>Fusobacterium, Bacteriodes,&nbsp;Prevotella, Propionibacterium, Peptostreptococcus, Porphyromonas<\/em>), staphylococci, streptococci<\/li><li>insects\/spiders \u2013 <em>S. aureus, S. pyogenes<\/em><\/li><\/ul><\/li><li><span style=\"text-decoration: underline\">signs\/symptoms<\/span>: pain and cellulitis at site of bite; swelling, purulent discharge<\/li><\/ul>\n\n\n\n<p><strong>(b) Necrotizing Fasciitis<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>infection that causes progressing destruction of fascia and overlying fat<\/li><li><span style=\"text-decoration: underline\">predisposing factors<\/span>: trauma, surgical <\/em>anaerobes<em>, V. vulnificus, E. coli, A. hydrophila, Porphyromonas.<\/em><\/li><li>many infections are polymicrobial involving aerobic-anaerobic bacteria (e.g., Clostridia + coliforms)<\/li><li><em>Vibrio vulnificus <\/em>and <em>Aeromonas hydrophila<\/em> usually waterborne infections due to traumatic wound injury (fish hook, fin stab) and preexisting open wounds, particularly in immunocompromised persons.<\/li><\/ul>\n\n\n\n<p><strong>(c) Burn Wound Infections<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>burns are skin damage caused by chemicals, electricity, heat, sunlight or radiation<\/li><li>three types of burns:<ul><li>first-degree = skin damage confined to epidermis<\/li><li>second-degree = epidermidis + dermis<\/li><li>third-degree = epidermidis + dermis + underlying tissues<\/li><\/ul><\/li><li>the skin is a part of innate immunity and contains macrophages to intercept invading pathogen<\/li><li>loss of the skin barrier allows microbes to feast on expose dermal tissue and potentially enter the blood stream<\/li><li>species often involved in burn wound infections are Staph\/Strep and <em>P. aeruginosa<\/em> (Silver sulfadiazine\/Silvadene covers)<\/li><\/ul>\n\n\n\n<p><strong>(d) Diabetic Foot Infections (DFI)<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>diabetes patients have a high rate foot infection due:<ul><li>loss of pain sensation (neuropathy)<\/li><li>reduced blood circulation (ischemia)<\/li><li>decrease neutrophil immunity and<\/li><li>weakened tissues<\/li><\/ul><\/li><li>due to the neuropathy and weakened tissue, the feet (usually on plantar surface) are more susceptible to trauma and open wounds may go unnoticed if pain is not felt<\/li><li>opportunistic bacteria can colonize the open wounds and the infection may advance to a serious disease (e.g., osteomyelitis) before recognition by the patient<\/li><li><span style=\"text-decoration: underline\">signs\/symptoms<\/span>: non-healing ulcers, edema, pus, foul-smelling odor (fermenting anaerobes present), cellulitis, abscess<\/li><li><span style=\"text-decoration: underline\">clinical microbiology<\/span>: necrotic tissue is removed; swab\/scrape base of the ulcer\/lesion or aspirate pus; specimen evaluated by Gram-stained smear and cultured under aerobic\/anaerobic conditions.<\/li><li>pathogens &#8211; polymicrobial, Gram-positive cocci: <em>S. aureus, S. agalactiae, <\/em>and <em>S. pyogenes<\/em> implicated most often in mild DFI.<ul><li>moderate infection \u2013 ibid plus Gram (-) aerobes (e.g., <em>E. coli, Proteus spp., Klebsiella spp.<\/em>)<\/li><li>extensive and systemic \u2013 ibid plus anaerobes (<em>Bacteriodes,<\/em> <em>Clostridium,<\/em> <em>Peptostreptococcus<\/em> spp.)<\/li><\/ul><\/li><li><span style=\"text-decoration: underline\">epidemiology<\/span>: 55K diabetes patients per year will have lower extremities amputated due in part to unsuccessful antibiotic therapy<\/li><\/ul>\n\n\n\n<p><strong>(e) Pressure Sores<\/strong><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>occurs most often in elderly, bedridden, and immobilized persons. Pressure sores (bedsores) occur in any location of body receiving inadequate oxygenated blood supply (ischemia).
